Two of Grinnell’s (Iowa) top professional organizations have partnered to coordinate services. The Mayflower Community’s residents and staff will receive therapy services from Total Rehab Orthopedic & Sports Specialist, P.C. Under the direction of Total Rehab’s owner, Stacey Alberts, PT/OCS/CSCS, therapy services are led by:
- Physical therapist, Dr. Jordan Finch-Smith, DPT
- Occupational therapist, Brittany Sleeuwenhoek, OTR/L
- Speech therapist, Lissa Ohrt, SLP
They are working with Mayflower residents and staff as well as others in the community in Mayflower’s Health Center, 600 Park Street, or in the Total Rehab offices, 234 West Street South Suite #4, Grinnell IA 50112.
Total Rehab can provide Medicare Part A (skilled care) and Part B reimbursed services. For anyone living independently in the Grinnell community, services can be initiated directly with Total Rehab for an evaluation without a physician referral.
Appropriate symptoms for therapy referrals include pain management, generalized or lower extremity weakness, pre- and post-surgery therapy, dizziness or vertigo, cognitive evaluations, and other issues with daily living activities. Total Rehab specialties include assistance with hand and gait issues, continence, injury recovery, and stroke rehabilitation.
